HELL ON EARTH

Marburg is one of five of the genera, filovirus. Ebola is a close nucleotide cousin. Filoviruses infected cells by various uptake mechanisms.

Following fusion of the intersticing membranes, the cell walls, the nucleocapsid is released into the receptor cell’s cytoplasm. This primary transcription instructs the cellular translation machinery to produce more viral proteins, the Cas9 sequencing. These are to be the templates for the production of millions of new viral genomes. Secondary transcription produces crRNA for synthesis of virion progeny. Assembly of new nucleocapsids occurs at the budding site—where the virus first entered the cell—and erupts into the host’s bloodstream. The cells have been recruited as “transformers” to recreate billions of virions. The host will die within ten days of infection. The virions will spread via bodily secretions of saliva, vomitus, feces, blood, tears, and semen. The process repeats millions of times—for each infection, each death.

As each believer prepared for his departure home, he began to feel weary. Marburg II symptoms include fever, chills, and muscle aches, much like the common cold. These were easy to ignore—and impossible to medicate. Surely, this was the emotion draining from them after such a glorious hajj. Surely, they would soon see the consequences of their pilgrimage. Surely, their impact upon the world of Satan was imminent.

Of course, they were right. Just in exactly the opposite manner.

Siddiqi coughed all night on the flight back to Damascus. He thought the wind must have given him the trouble, but he wondered.

Why do I have blood in my stool? Why is my head pounding? The blessings of Allah are hard to understand.

The incubation period could be very short. Each messenger’s illness came on during his flight home, sickening many other passengers. Three of the flights were quarantined upon arrival as the Marburg beast emerged quickly from the sands.

These efforts were futile. Quarantine stations in Sudan, Mali, and Tajikistan simply meant waiting inside the terminal for a doctor. When no one came, the cordinaires simply walked home.

Marburg II began abruptly, with high fever, severe headache and malaise. Muscle aches and pains were common. Watery diarrhea, abdominal pain and cramping, and nausea and vomiting could begin on the third day. Diarrhea could persist for a week. Patients appeared ghostly, with drawn features, deep-set eyes, expressionless faces, and extreme lethargy. A nonitchy rash appeared in most patients between two and seven days after onset of symptoms.

Many patients developed severe hemorrhagic manifestations between five and seven days, usually with some form of bleeding, often from multiple areas. Fresh blood in the vomit and feces was often accompanied by bleeding from the nose, gums, anus, and vagina. Spontaneous bleeding occurs at venipuncture sites, where intravenous access was obtained to give fluids or obtain blood samples. This could be particularly troublesome to health-care workers.

During the severe phase of illness, patients sustained high fever. Breakdowns within the central nervous system could result in confusion, irritability, and aggression.

Death occurred between eight and nine days after symptom onset, usually preceded by severe blood loss and shock.
